The registration deadline is August 15, 2026.
Each applicant must submit both a registration and medical form. Click on each form then download, open on your computer to fill out the forms, sign, and mail them back before the registration deadline. Please note: Include your physician’s name and contact information when completing the medical form; however, the physician’s signature is optional.
